The Investigative and Dictaminating Commission of Aviation Accidents and Incidents, which carried out the technical investigation of the accident of the Agusta A109S helicopter, which occurred on December 24, 2018, in Santa María Coronango, Puebla, and in which the then-governor lost their lives of that entity, Martha Erika Alonso, her husband, Rafael Moreno and three other people, were the product of a combination of factors, such as lack of maintenance of the aircraft and loss of control during the flight.

At a press conference, the Secretary of Communications and Transportation, Javier Jiménez Espriú, reported that the agency's investigators and the authorities of the countries that manufactured the helicopter parts confirmed that during the inspection of the remains, no indications of materials were observed. oblivious to the aircraft, there were no bird pieces or projectiles, nor evidence of damages that prove that there was a possible act of sabotage or explosives.

He stated that the Final Report and its annexes will be forwarded to the Office of the Attorney General of the Republic, since it is a federal investigation, so that they can be included in the file that has been started, in order to continue with the pertinent investigations; It will also be sent to the Attorney General of the State of Puebla, for the corresponding legal effects.

He noted that among the immediate actions to be carried out by the Federal Civil Aviation Agency are to carry out immediate audits of the operating company Servicios Aéreos del Altiplano SA de CV, and the maintenance workshop Rotor Flight Services, S. de RL de CV, temporarily suspending their operations, until they have the result, to take the definitive actions.

The Final Report, which was presented by the head of the SCT to President Andrés Manuel López Obrador, is the result of 15 months of hard work, research, analysis, and tests carried out by specialists both in Mexico and in the United States, Italy and Canada, and details the tests that were performed on each of the components of the aircraft, said the official.

The document establishes that: “Although the information available for the investigation was limited, due to the lack of flight recorders protected against impacts and the severe damage to the helicopter due to the impact against the terrain and subsequent fire, based on the available evidence, it was determined that the sudden roll to the left was due, possibly, to the fact that both linear roll actuators were extended simultaneously, without having been commanded ”.
